Medical Services and Transportation and Travel
JoshProvides offers Medical Services and Transportation and Travel grants for diagnostic testing, specific safety items, and transportation & travel for diagnostic testing. These grants must be used within a three-month period from the date of the grant award. These grants are date specific and must be indicated on Appendix D by your neurologist. Diagnostic testing must be scheduled at least three weeks from the application submission date. All payments are made directly to the physician, facility or vendor. Patient/Applicant may only submit one Medical Services and Transportation and Travel Application once in a twelve-month period. Funding is not available for medication or prior services and outstanding balances.
